Although there are more than 2 mistakes, the 2 most common are:
1. Not wetting the Line before tightening
2. Not pulling both tag end and main line
The other not mentioned is not obvious and needs to be learned through experience, and that is the number of wraps based on line diameter. Too little line would slip, too much would not tighten properly, therefore it will loosen up and slip.
Make sure the knot is tight before you cast! if its loose.. it will continue to loosen up and slip!
